Eligibility Requirements:

• Florida-based policyholder
• Within 60 days of renewal
• Homeowners or small business policy
• Willing to complete structured feedback

Participation Terms:

• Required policy documents must be submitted within 48 hours of receiving secure portal access
• The delivery timeline begins once all required documents are received
• The completed Report will be delivered within 5–7 business days
• A short structured feedback form must be completed within 72 hours of delivery
